I am having a problem with extra HTML code showing up in the feedback textbox when I am grading an exam. Whenever I go to Utilities --> View, Grade, or Delete Submissions --> Grade (by someone's name)... i see the questions of the exam, as well as extra code in the feedback box (see attached image). I am having to delete it for each student and then type in my comments. Anyone know how to prevent this code from showing up?

Firefox extension
I've seen this before. It's one of the extensions installed in Firefox. I believe it's one called "Browser Highlighter."
Please go to Tools>Addons and either disable or uninstall this extension. Restart Firefox and see if it recurs. If it does, please run Firefox in Safe Mode and see if it recurs. If it it does, do let us know.
If not, try disabling your extensions one at a time, then restarting, then try to reproduce the issue. If you track it down to a different extension, please comment with its name—that information would probably be very helpful to someone else in the future.

I don't seem to have that extension loaded. This problem also occurs if I use Internet Explorer, so I don't think it is confined to just Firefox.

Hm. That text is absolutely not generated by ANGEL and is generated by the Browser Highlighter extension. Apparently it's bundled with Skype and people may install it without realizing it. Please see the comments on the extension's page.
Another possibility for why you are seeing this text is that someone else who created or edited the quiz in question had the Browser Highlighter extension installed. When they created or edited the quiz questions the extension inserted the text that you see into the feedback fields. When the person saved, that text was saved into the field. Since it's the default feedback for the quiz you and all other course editors/assistants will now see it when grading that quiz.
It's also possible that it's generated by something else installed on your computer that's capable of hijacking both IE and Firefox and inserting that text. I think the previous suggestion is more plausible, however.
I'd like you to try something. Please go into another course or group—preferably one that has no students enrolled in it and is not being used for course delivery. Create a new quiz and add a few questions to it. Take the quiz, and grade it. Does the problem recur?
If it does not recur then the situation described in my first paragraph is the culprit. Go into the original quiz, click edit next to one of the affected questions, and look at the feedback field. It will likely have the offending text in it.
I strongly recommend going through all of the all of the content in the course, checking for this problem, and removing the offending text. Wherever the HTML editor appears, click the Source button. Clean out the offending text wherever you see it. I'd also recommend contacting everyone who is an editor in the course and advise them to check for and remove the offending Firefox extension from their machines.
